A service coordinator is a unique personal advisor, much like a concierge, that acts as a trusted advocate for you and your family. They provide support and assistance and connect you with important community resources and information. Whether that’s help understanding your Medicare insurance, getting help with transportation to physician’s appointments, finding health care providers, or a local quilting club, your service coordinator can help you get back to enjoying the things you like to do without worrying about the logistics of everything else. Service coordination boosts your independence by offering support as you need it, how you need it.
Service coordinators are available to help residents in many UCH communities, including at the new United Church Homes – The Polaris Community. Service coordinators will also be available to older adults and their families throughout Delaware and Franklin counties in early 2022.
“We understand the trusted support and services our residents rely on can extend beyond the front doors of our communities and into surrounding neighborhoods,” said Dan Fagan, MPA, LSW, UCH executive director of housing and services.
“Aging abundantly means looking at all factors that contribute to health and longevity,” said Fagan, “It’s your access to healthy food, reliable transportation when you need it, and social experiences that enrich your well-being. It’s about wrapping our arms around older adults and meeting all of the realities of life with them.”
The difference between service coordination and other senior living programs is the personalized experience. Service coordinators maintain an extensive network of agency and community contacts they work with regularly to assist older adults.
Olivia Markley, service coordinator at The Polaris Community, knows first-hand how aging is unique to everyone. “My goal is to be an advocate, resource, and trusted friend for our residents and their families, no matter what they may need,” she said.
“I connect residents to local senior center programs like SourcePoint and help enroll them in prescription management services, so their medications are organized. There are so many resources in the Columbus area for older adults. My job is to help residents discover what they need and initiate connections to help them live worry-free lives.”
