Summary:
The Provider Relations Specialist is responsible for fostering and managing relationships with contracted service providers and partners, monitoring provider compliance, and supporting quality improvement initiatives. Working under the supervision of the Quality Improvement Manager, this role includes oversight of provider performance, administration of service agreements, and facilitation of training and support to enhance provider understanding of compliance requirements and best practices.
Essential Job Functions:
Provider and Service Contract Oversight
- Cultivates and maintains strong relationships with providers and partner organizations to ensure effective collaboration; serves as the primary liaison to address concerns, facilitate communication, and support partnerships.
- Monitors In-Home Care provider compliance with all conditions, specifications, and policies, evaluating provider performance and offering feedback to the team on any corrective actions for noncompliance; tracks resolutions as needed.
- Oversees, tracks, and evaluates partner contracts to ensure alignment with organizational standards.
- Manages and initially approves billing for In-Home Care Services providers and some contracted community partners.
- Assists external partners with billing, including entering service units into the In-Home Care Services case management software.
- Supports the Quality Improvement Manager in reviewing and updating provider application materials, including Conditions of Participation and Service Specifications; communicates updates effectively to providers and staff to ensure alignment and understanding.
- Collaborates with the team to identify service gaps and address these by developing new services or non-provider contracts for In-Home Care Services.
- Provides technical assistance and training sessions for In-Home Care Services providers; works with providers, client services management, IT, and fiscal staff to develop training materials and conduct formal explanations of all applicable rules, guidelines, and requirements for contracted providers and provider applicants.
- Guides new providers through the onboarding process, setting up initial compliance checks and familiarizing them with requirements.
- Addresses and mediates conflicts or disputes with providers, aiming to resolve issues proactively before escalating.
- Tracks provider performance metrics and prepares reports to support ongoing quality
- improvement initiatives.
- Collaborates with other departments, such as finance for billing processes and marketing for partnership communications, to ensure cohesive provider relations across the organization.
- Assists in managing provider relations during emergencies or crises, ensuring continuity of care and support for providers in these situations.
Grant Management
- Monitors and tracks grant reporting to ensure ongoing compliance with established goals and adherence to approved budgets.
- Schedules and conducts on-site program reviews to assess alignment with programmatic objectives.
- Supports the annual grant invitation-to-bid process by maintaining effective communication with applicants, updating and managing key documents, providing feedback on current grant recipients, and preparing summaries and notifications.
General Responsibilities
- Actively participates in SourcePoint required meetings and events.
- Pursues and attends training sessions related to professional development in the provider relations role.
- Maintains a thorough understanding of all SourcePoint programs and services, as well as an understanding of the programs supported through partnerships.
Education & Experience:
- Bachelor’s degree in Social Work, Public Administration, Health Administration, or a related field required.
- Minimum of 4 years of experience in provider relations, case management, compliance, or a related field.
Required Skills & Abilities:
- Knowledge of geriatric care, unique needs of older adults, and local resources available to support their health and well-being.
- Knowledge of best practices in program and social service administration, including service coordination, case management, program delivery and oversight, referral principles, and effective resource management.
- Strong communication skills, including reading comprehension, professional writing, group facilitation, presentations, and handling difficult conversations with various individuals.
- Demonstrates strong attention to detail and proficiency in managing complex compliance and reporting tasks with high accuracy.
- Proficiency with Microsoft Office products and other basic software.
- Excellent organizational skills with the ability to handle multiple tasks while meeting deadlines.
- Ability to work cooperatively with diverse groups of professionals.
- Ability to adapt to change and ambiguity on a routine basis.
- Strong problem-solving skills with the ability to analyze issues and present information to influence the opinions or actions of others.
- Ability to promote a positive work culture both within the organization and in the community.
Essential Physical Requirements/Working Conditions:
- Must be able to lift and/or exert up to 20 pounds regularly without assistance; ability to navigate stairs.
- Requires a valid driver’s license and flexibility for off-site meetings, events, and occasional evening meetings.

In a small organization, team members may need to assume additional responsibilities to ensure smooth operations. Other organizational needs may arise from time to time, which will be assigned to the Provider Relations Specialist as appropriate.
