Caring for a spouse, parent, or loved one takes a great deal of time, effort, and hard work. Sourcepoint’s caregiver support offers a variety of free resources and dynamic programs. We are looking for a Social Worker-Program Coordinator to oversee this invaluable service to our community in Delaware County.
Summary
The Social Worker-Program Coordinator is responsible for coordinating, evaluating, and expanding our comprehensive caregiver support program. The program will include a broad array of activities and services that support, guide, educate, and address the needs of caregivers served by SourcePoint.This position has general flex office hours between 8 a.m. and 5 p.m. Monday through Friday. The position requires approximately 1 weekend a quarter and 1 to 2 evenings per month.
Essential Job Functions
Caregiver Program Creation, Maintenance & Facilitation (50%)
- Identify, suggest, and develop tools and activities to be implemented as part of a comprehensive caregiver support program, requiring contact with the community, employers, staff, caregivers, respite providers, and volunteers
- Facilitate professional-led support groups for caregivers throughout Delaware County monthly
- Coordinate and assist with peer-led support groups for caregivers monthly
- Present and regularly update the Caregiver Crash Course, a 30 to 60-minute class and video for caregivers
- Plan and lead evidence-based program(s) designed to enhance caregiver self-efficacy and self-care, several times per year, such as Powerful Tools for Caregivers and Aging Mastery Program for Caregivers
- Coordinate the Caregiver Relief Companions Program including assisting with the recruitment and training of volunteers, working with care consultants to identify client referrals, and matching respite volunteers to clients for service
- Plan and implement caregiver appreciation events each November (Caregiver Appreciation Month)
Caregiver Contacts (15%)
- Send emails, at least monthly, to active SourcePoint caregivers to stay connected and ensure they are aware of upcoming programs and caregiver resources
- Be consistently prepared and available to talk with and provide active listening to caregivers in one-on-one supportive conversations in person or over the phone
Grief Support Program Creation, Maintenance & Facilitation (15%)
- Facilitate Caregiver Grief Group, a semi-annual 6-week, guided book club-style group for former caregivers
- Facilitate Living Beyond Lose, a monthly dynamic support group for those dealing with loss
Documentation & Reporting (10%)
- Track and report all caregiver/former caregiver data necessary to meet the requirements of funders and the needs of management and staff
Community Relationships (10%)
- Maintain a thorough understanding of programs and services provided and supported by SourcePoint. This includes SourcePoint programs and services, as well as those supported through our partnerships
- Maintain partnerships with off-site program locations and other caregiver-related entities in the Delaware County community
- Work with partners to offer fun and educational activities for caregivers and their loved ones annually
Required Skills and Abilities
- Knowledge of geriatric care, older adult issues, family caregiver issues, and local resources available to older adults in the support and maintenance of their health and welfare
- Knowledge of training techniques and workshop development skills and experience in coordinating and facilitating training, education, and technical assistance with individuals and groups
- Excellent computer skills, including fluency with basic software products, including Microsoft Office
- Strong ability to work cooperatively with a diverse group of clients, caregivers, volunteers, coworkers, human service professionals, and organizations as well as navigate group dynamics
- Ability to meet deadlines and successfully manage multiple tasks simultaneously
- Ability to maintain accurate records and prepare professional correspondences and reports
- Ability to promote a positive work culture within the organization; project a positive image of the organization in the community and routinely interact with the public
- Must have reliable transportation to and from work, offsite events, etc., as assigned
- Ability to be creative and deal with ambiguity and change on a routine basis
- Serve as person-in-charge per schedule
Education and Experience
- Associate degree in social sciences with 5 years experience or bachelor’s degree in social work required
- A registered social work assistant, Licensed Social Worker (LSW), or counselor (LPC) required
- Experience working with older adults or other vulnerable populations preferred
- Experience with support group facilitation, and/or program coordination preferred
Essential Physical Requirements/Working Conditions
- Able to occasionally move about the building and operate office machines
- Able to remain in a stationary position 50 percent of the time
- Able to communicate clearly in group settings, 1:1, and presentations, via in-person, phone, and virtually
- Able to drive SourcePoint’s mini cargo van
- Must be able to lift and/or exert up to 20 pounds occasionally

In any organization, particularly a small organization, it is necessary to assume new responsibilities to ensure a smooth continuity of operations within the organization. It is expected that other organizational needs will emerge, which will be assigned to this position.
