PRESS RELEASE– SourcePoint has once again earned the highest level of accreditation from the National Committee for Quality Assurance (NCQA) for its in-home care program. NCQA awarded SourcePoint a three-year Accreditation of Case Management for Long-term Services and Supports (LTSS), the organization’s second time receiving this top distinction.
NCQA’s Case Management Accreditation for LTSS is a comprehensive, evidence-based evaluation of case management programs serving individuals in community-based settings. Achieving this accreditation demonstrates SourcePoint’s commitment to delivering coordinated, person-centered, and integrated care that supports individuals in functioning safely and effectively in the setting of their choice.
SourcePoint’s in-home care program helps Delaware County adults age 55 and older remain in their homes with independence, dignity, and safety. Services include emergency response systems, home-delivered meals, medical transportation, personal care, and additional support tailored to each individual’s needs. Care is overseen by licensed social workers and counselors who develop and monitor individualized care plans.

SourcePoint’s Case Management for LTSS Accreditation is valid through December 2028.
