Application for Employment

I certify that the facts contained in this application are true and complete to the best of my knowledge and understand that, if employed; falsified statements on this application shall be grounds for dismissal. I authorize investigation of all my statements contained herein and references, employers listed above and shared by other means, to give you any and all information concerning my previous employment and any pertinent information that may have, personal or otherwise, and release the company from all liability for any damage that may result from utilization of such information. I also understand and agree that no representative of the company has any authority to enter into any agreement for employment for any specified period of time, or to make any arrangement contrary to the foregoing, unless it is in writing and signed by an authorized company representative.

I authorize the process of the background and driving record check, and that all company property such as keys, key fob, materials and equipment, uniforms, etc. must be returned immediately when requested.

I give permission to SourcePoint, or any of its media partners to use my picture, portrait, photograph, image, or voice in any of all forms of marketing and advertising. I waive any right to inspect or approve the finished product, including written copy that may be created in connection therewith. I also agree that this releases SourcePoint or its representatives from any and all monetary obligations or payments to me or any or all of my authorized representatives for use of video, films, photographs, image and /or voice of myself.
