SourcePoint’s in-home care services are designed to help Delaware County adults 55 and older remain living safely in their own homes with independence and dignity. We provide direct access to in-home services, as well as referrals to other community resources that can help. Eligibility for in-home care services is based on need, not income, and services are available on a sliding fee scale.
The Three C’s of In-Home Care
Care
Our licensed social workers and counselors, called care consultants, work one-on-one with each client to customize a plan of needed services. They specialize in issues associated with getting older.

Cost
SourcePoint’s services are available to all Delaware County residents age 55 and older. Eligibility is based on need, not income, and services are available on a Sliding Fee Scale, so copays differ for each client.
What We Do
Provides socialization, support, and safety to individuals who are chronically ill or have a disability, while allowing them to return home each day.
Help with a variety of chores, such as changing light bulbs, tacking down cords or rugs, raking leaves, shoveling snow, and more.
A small help button, typically worn around the neck or the wrist, provides a link to 24-hour emergency assistance.
For those unable to perform household tasks, services may include light cleaning, cooking, and laundry. Homemaker escorts also available to accompany you on errands.
Receive delicious hot and cold meals delivered by a caring volunteer. We offer several menu options, including meals with restricted carbohydrates or low sodium. LEARN MORE
In-home care clients can receive rides to medical appointments, social service agencies, and adult day care centers.
In-home behavioral health services are available to those enrolled in our program.
Registered nurses help clients achieve optimal health and well-being through simple screenings and medication set-up.
Personal care assistants or home health aides help with daily activities like bathing and dressing.
Respite services are designed to provide relief and rest for family caregivers, so that stress and tension is minimized.
